Please complete the following form to apply to Coach U8 or U9 MD teams for the 2023-24 Season.

Coach Applicant Information

2023-24 ALLIANCE Coaching Requirements:

Head Coach
 - Coach 1 Intro to Coach (U9 and below) OR Coach 2 - Coach Level (U10 and above) certification, up-to-date Vulnerable Sector Police Check (or Declaration form for 2nd & 3rd year), Respect in Sport- Activity Leader (for Bench staff), Gender Identity and Expression Course (to be completed online upon award of position)

Assistant Coach - up-to-date Vulnerable Sector Police Check (or Declaration form for 2nd & 3rd year), Respect in Sport - Activity Leader (for Bench staff), Gender Identity and Expression Course

Trainer - HTCP - Level 1, up-to-date Vulnerable Sector Police Check (or Declaration form for 2nd & 3rd year), Respect in Sport - Activity Leader (for Bench staff), Gender Identity and Expression Course

Manager - HTCP - up-to-date Vulnerable Sector Police Check (or Declaration form for 2nd & 3rd year), Respect in Sport - Activity Leader (for Bench staff), Gender Identity and Expression Course

On-Ice Helper - HTCP - up-to-date Vulnerable Sector Police Check (or Declaration form for 2nd & 3rd year), Respect in Sport - Activity Leader (for Bench staff), Gender Identity and Expression Course

ACKNOWLEDGEMENT & CONSENT

By completing and submitting this Application Form, I hereby acknowledge and agree as follows:

Bench Staff Requirements
At least one (1) member of my bench staff MUST be a certified Trainer.

All proposed bench staff must also complete and submit their own application referencing my application.

Further Certifications
I, and/or members of my bench staff, m ay be required to attend coaching clinics and obtain further certifications depending on the nature of my and/or their position.

Police Record (Vulnerable Sector) Checks
For the safety of all participants, I must obtain a Vulnerable Sector Check from the Waterloo Regional Police Department (which can be ordered online form the WRPD website) and submit the completed Vulnerable Sector Check to Waterloo Minor Hockey.  If I am a returning volunteer, I must sign and submit an annual declaration to Waterloo Minor Hockey and obtain a new Vulnerable Sector Check every three (3) years.  I will ensure that a valid Vulnerable Sector Check is on file with Waterloo Minor Hockey prior to the start of the season.

Certifications & Authorizations
I understand that completing and submitting this Application does not ultimately guarantee me a coaching position with the Waterloo Minor Hockey Association.
