Please ensure that everyone that is attending the U-8/2013 MD evaluations is properly registered within the "519Check-in App for COVID-19 Symptom Screening", you need to ensure that the participant is registered under the noted group below.
"MD - U8 (2013) - Evaluation - Group A or B" for every evaluation outlined within the team calendar on the WMHA MD website. You must outline the participants by name, and confirm that they have no COVID-19 symptoms prior to attending.
This is a mandatory requirement to enter the facility and participate.
