    Reimbursement Request - Required Courses
    Please use this form to submit a reimbursement request for required coaching courses in order to be approved on a roster with your team. This would include the charge to obtain a VSC from Police Services.

    WMHA Suspension Reporting Form
    Please use this form to report ANY suspension received during an exhibition, league (including playoffs and finals) or tournament game.

    Meeting Room Booking Request
    Please complete this form to request a meeting room to be booked for your team. Please remember, the WMHA requires at MINIMUM - 48 hours notice to book the room (not including weekends or holidays). The WMHA will cover the cost of one meeting room (max. 2 hours) for each team, per season. After the 1 x booking, your team will be invoiced for the cost of any rooms booked in December and April of each season.
