2024 F1 AAA Waiver Request Form

Please complete this form to request an F1 Waiver from Waterloo Minor Hockey. **PLEASE NOTE: this form will be active for players in the U10 to U13 age group on April 20, 2024 (NOT THE U14, U15 or U16 age group)** **PLEASE NOTE: this form will be active for players in the U14 & U16 age group on April 22, 2024. **PLEASE NOTE: this form will be active for players in the U15 age group on April 23, 2024

To be eligible for a “AAA” Waiver, a player must register for and attend “AAA” tryouts in their appropriate age division and not have signed or been offered a card to sign with a Waterloo Wolves team.

For an adjacent AAA club/association/zone team to permit a non-residential player to tryout, the player must provide a copy of the Electronic “AAA” Waiver (may be presented in printed or electronic format).

All electronic “AAA” Waivers will be EMAILED to the parent/guardian of the player, as well as the affiliated member “AAA” Waiver delegate, and will include the subject line of “AAA” Waiver - Player Name.

Parents/Guardians are NOT permitted to request a F1 Waiver until the Coach has released the player.

F1 Waivers will NOT be granted until confirmation from the Coach has been received by the F1 Waiver delegate.
