
**UPDATE TO SCREENING UPON ENTRY TO CITY OF WATERLOO FACILITIES - PLEASE READ**
Starting
Monday December 6, The City of Waterloo will health screen, collect contact tracing
information and check vaccine status (as applicable) for ALL those who enter a
City of Waterloo facility, regardless if these measures have taken place
through the sport organization or rental organizer.
ALL PARTICIPANTS (players, parents, coaches, etc.) may either complete the City’s health screening and contact tracing form in advance at www.waterloo.ca/covid-screening (printable pdf) or at the door by answering the questions and providing contact information at upon arrival.
Parents are encouraged to complete the health screening and contact tracing form in advance if someone other than themselves are dropping off children at a facility.

At this time, we would also like to remind all Waterloo Minor Hockey Association Members that masks are to be worn in all common areas (including hallways and spectator areas) and to continue to be respectful City and Security Staff.
