OPTIONS TO PROVIDE PROOF OF VACCINATION/EXEMPTION

ALLIANCE HOCKEY - COVID VACCINE POLICY REMINDER

ALLIANCE Hockey will require proof of COVID-19 immunization for all vaccine eligible participants(12 years of age and above and unless exempted) including players, team officials, game officials, staff, and volunteers in order to participate in ALLIANCE Hockey sanctioned programming.




Eligible participants will require proof of having received their first dose by SEPTEMBER 17, 2021 and be fully vaccinated by OCTOBER 31, 2021 OR provide an approved vaccination exemption form, from ALLIANCE Hockey. 

We are offering TWO methods to provide this information: 

1) By online form from our website. 
ONCE DOCUMENTATION IS REVIEWED, IT WILL BE DELETED.

For Coaches/Bench Staff/Volunteers, click HERE 

OR

2) In person to one of our Association Representatives - we will need to see the applicable individual's (ie - player or Coach's) Ontario Health Card and Dose Administration Receipts (if you have received 2 doses, we only need to see the receipt that indicates the 2nd dose). WE DO NOT NEED TO KEEP ANY PAPERWORK AND WILL RETURN IT TO YOU IMMEDIATELY AFTER REVIEW. DIGITALS COPIES ARE ACCEPTABLE. 

We will have several dates where our Association Representatives will be available in person to review your documentation and will post them on our website - our next date is: 

FRIDAY, SEPTEMBER 17th
RIM PARK - OUTSIDE (TO THE RIGHT SIDE OF THE ENTRANCE A)
5:00-8:00pm
**please remember to bring your Ontario Health Card**
