WMHA Coaches - Please use this form to submit the names and contact information of your support Staff for the season. This form does not include your coaching or training staff, as we have captured their information in another manner. Please also provide us with the etransfer email address, should you be making or receiving payments, via this method.

Please provide the name and contact email for your Team Webmaster - this would be an individual who would like to have access to your team site to post articles, pictures, calendar events, etc.

ADDITIONAL CONTACTS

Please use this area to submit the name, title and contact email for anyone else you would like us to know about, for example - Social Committee Chair, to book a party room on your team's behalf.
